Picture a five person architecture practice. Everyone takes notes on site: half a page in a notebook, a voice memo in the car, three photographs with no caption.
Back at the office none of it is usable by anybody else. The person who went knows what "check the lintel, G said fine but see photo" means. Nobody else does, and in three weeks neither will they. Actions get missed, and the ones that get remembered get remembered by whoever happened to be there.
The tempting fix is a better template nobody fills in. The more useful move is to treat it as a tool decision: what is the job, what would good look like, pick one thing, and check in a month whether it actually helped.
