Human in the Loop

In short
A person who checks the output before it counts.
In plain English
A deliberate checkpoint where a person reviews, approves or corrects what the AI produced before it reaches anyone else. The point is not distrust; it is designing for the failure mode.
A real example
An AI drafts every support reply; a person reads and sends. You get most of the speed and none of the risk of a confidently wrong answer going out under your name.
Why it matters
It is the single most effective safeguard available, and the first thing dropped when a team gets busy. Decide in advance which outputs require it, and write that down.
